  Reasons for Requesting a Home Visit:

• Too elderly and frail to come to the surgery

• Housebound with severe illness

  Remember:

• It is always better to come to the surgery if possible as we can meet your medical needs more easily from a purpose built surgery than in your home

• In a true emergency dialling 999 for the Ambulance service will provide more rapid and appropriate care

• If you can get to the surgery you will be seen sooner than if you wait for a visit

  Assessing your Needs:

1. Home visit requests can be made at the Reception desk by a person acting on behalf of the patient, or by telephone

2. Please request visits, if possible, before 1030am as this will enable the doctor to arrange visits in the most productive way

3. One of our reception staff will record your request

4. Your doctor will contact you to assess the situation

5. You may be offered telephone advice or asked to visit the surgery

6. You will be visited at home if your doctor feels this is appropriate
